The Admission Process
We want the path into Melrose Residential to be clear from the first conversation. Here is what to expect once we are open and accepting residents.
Melrose Residential is not yet accepting residents or referrals. This page describes how admission will work once we are licensed and open.
Six steps from first contact to move-in
Step 1: Reach Out
A prospective resident, a family member, or a referring agency contacts us. We listen, answer questions, and learn a little about the person and what they are looking for in a home.
Step 2: Share Information
With the resident's or legal representative's permission, we gather the background we need, such as referral details and information from their current providers.
Step 3: Screening
We offer every prospective resident a screening meeting with our Administrator, where you can learn about Melrose Residential and we can learn about you. The Administrator reviews the screening information to decide whether the person meets our admission criteria.
Step 4: Our Decision
We let the person and their referral source know the decision. If we can’t offer admission, we explain the specific reason in writing, share the right to appeal, and help point toward other options. If we’re full, you may be placed on our waitlist.
Step 5: Review the Agreement
We go over the Residency Agreement together, including room and board, what is included, resident rights, and the reasons a residency could end and how to appeal. You can bring a family member, friend, or representative to help, and you receive a copy of the signed agreement.
Step 6: Move In
Once you’re ready to move forward, we set a move-in date and help you get settled in your own bedroom. You are welcome to bring your own furniture and make it yours. Soon after you move in, we write your service plan with you.

What to Bring
Once you have a move-in date, our team will help you get ready ahead of time. If you can't gather everything at once, that's okay. We'll help you find what's missing.
You're welcome to bring your own furniture and decorate your bedroom. We'll inform you of what fits in the room before you move in.

What We Provide
We provide a bed, a dresser, locked storage, bedding, towels, and basic hygiene items to help you feel comfortable and prepared for your stay.
Packing Checklist
- Valid photo ID
- Insurance or benefits card
- List of current medications
- Prescriptions in original pharmacy bottles
- Comfortable clothing
- Any relevant medical or psychiatric records (if available)

Who can refer
Referrals typically come from a hospital, a county mental health program, the Oregon Health Authority or its designee, or another licensed residential program. Individuals and families are welcome to contact us with questions, and we’ll explain how to start a referral.
