Our Program

Melrose Residential offers a comfortable home, a warm and predictable routine, and a team that is on site around the clock. With only five residents, support is personal and fits the pace of each person

Melrose Residential is not open yet. This page describes the program we are preparing to offer once we are licensed by the Oregon Health Authority.

The program

How Our Program Works

Melrose Residential is a Residential Treatment Home in Portland for up to five adults who live with serious mental illness. Each resident has a private bedroom, as well as shares meals and common areas with the other residents. Residents also take part in life in the neighborhood and the wider Portland community.

Support is built into each day. Staff are on site around the clock to help with daily routines, medication support, and the practical side of running a household. Each resident helps write their own service plan, and we work with the providers they already see.

What residents receive

Services and Support

Melrose provides help with daily routines, medication support, activities, and care planning. Services are matched to each resident’s needs and written into a service plan that the resident helps create

    Daily support

    Our team will be on site around the clock. They will help with the everyday parts of life, like meals, appointments, and household routines, and be there on hard days.

    Medication support

    Staff help residents take their medications as prescribed, within what Oregon rules allow for a licensed residential treatment home. Prescriptions come from each resident's own prescriber.

    Skills practice

    Residents work on everyday skills they choose, with staff support and at their own pace. We will list the specific skills groups here once our program schedule is set.

    Coordinated care

    • With each resident's consent, communication with their psychiatric provider, case manager, and primary care doctor

    • Three nutritious meals each day, and access to food and snacks at any time

    • Utilities and laundry
    • Housekeeping of common areas

    Activities and community

    Residents are part of the neighborhood and the wider city. Outings and time in the community are part of the week.

Outside care

Working With Your Providers

Residents keep their own outside providers, which usually means a psychiatric provider, a case manager, and a primary care doctor. Residents who would like a different provider can choose one, and our staff can help them find one.

Residents can go out to their scheduled appointments, and where it can be arranged, a provider can come to the home instead. We do not replace any of these providers. With the resident's agreement, we stay in touch with them, share what we see day to day, and help everyone work from the same plan.

Your plan

Your Service Plan

Every resident has a written service plan. It sets out what the person wants to work on and the support they will receive from staff.

The plan is written with the resident, not for them. A family member or case manager can take part when the resident wants them to.

Plans are reviewed as things change. We will add the review schedule here once our policies are final.
